This national awards program recognizes the most innovative and effective work-based learning programs designed specifically for temporary and contract employees.
CHN was honored for its Managed Care Project in which CHN provides highly skilled and experienced Home Care registered nurses for a Boston area health plan to be cross-trained in performing new member Assessments and Care Management during peak enrollment periods. These nurses are able to quickly adapt to the environment, needs, and community resources available all with the goal of keeping patients in their homes and out of the hospital.
